How to Request Specimens

Download and complete our Specimen Intake Form and email it to us at SAN@fredhutch.org with "SAN Specimen Request" in the subject line. Please fill out the fields in as much detail as you are able.

Please email us with any other request or questions you may have.

Be sure to have the following documents and information available for the intake form, if applicable:

  • Target date for collection
  • Summary of proposed project
  • IRB/ERB approval # and date
  • Funding source
  • Data Sharing (intentions/requirements)
  • Specimen details, as relevant to request
  • Patient-level details
  • Associated clinical data request

Next Steps

You can expect a response to your request within 2-3 business days to confirm the request has been received and to follow-up with any additional information needed. Our Coordinating Center staff will reach out to our network partners to find the specimens that fit your needs.

When a source has been identified, we will provide you with information about approximate cost, timeline for receipt of specimens, and how to complete your request. We will provide templated material and data transfer documentation and consents to streamline the transfer process and address any questions you have along the way.

Once you make a request, you will work with the institutional contact to arrange payment and transfer of the specimens and any associated data that has been requested. Our team will be available for any additional support you need or questions you may have during this time.
